Voluntary Benefits Open Enrollment Period:   Sept. 26-Oct. 31, 2016

 

WHAT ARE THE KEY THINGS YOU NEED TO KNOW AS AN ACTIVE EMPLOYEE?
Your plan options will remain the same as 2016.   If you select the Aetna Dental DMO Plan you must select a primary dentist during open enrollment.   To have coverage starting January 1, 2017 and to avoid the $50 monthly Tobacco-Use surcharge, you must enroll online by the October 14th deadline.

If you fail to enroll, you and your current dependents will be defaulted to the Kaiser HMO for the 2017 plan year with no coverage for dental and vision.

HOW TO EARN/KEEP $240 WELLNESS CREDIT FOR 2017

You can reduce your 2017 premium under any of the medical plans by $20 each month by making an appointment to see your doctor OR attending a biometric screening if you are unable to see your provider. You must complete the requirements with the Medical Plan Provider (BCBS or Kaiser) that you are currently enrolled in by December 31, 2016.

HOW TO AVOID THE $50 MONTHLY SURCHARGE FOR 2017
Visit http://benefitenrollment2016.fultoncountyga.gov/apps/tba/ to complete the Tobacco-Use Attestation online, to avoid the $50 monthly surcharge effective January 1, 2017 if you have not done so already.
