If you mail in your request – it must include a letter giving us permission to process your request to also include: 
• full name, sex, race, date of birth, SSN #, 
• photo copy of your ID (ex. Driver’s license, passport, or state ID) 
• a self-address stamped envelope to where you want your request to be mailed
